About us

HN was founded in 2010 by clinicians and researchers from the Karolinska Institute, Sweden. The UK operations were established in 2015 and we are proud to have helped improve the lives of over 30,000 patients by working alongside them to achieve better health.

We specialise in the practical application of population health management, by identifying high-cost, high-need patients, and supporting them to improve their outcomes and reduce their care consumption. HN has worked with the Nuffield Trust on a randomised controlled trial of AI guided clinical coaching within the NHS with excellent results.

Our team

HN is comprised of professionals from a range of clinical, digital, data and business backgrounds. We are highly experienced in delivery of innovative clinical services, use of applied AI within healthcare, research and evaluation.

Most of the HN team have worked within the NHS so are familiar with its processes and all our patient facing services are delivered by trained and qualified healthcare professionals. Furthermore, we have an excellent Advisory Board and Regional Advisers who are senior stakeholders from within NHS commissioning, primary care, population health management, adult social care and research, that we actively work with to achieve our goals.

Creenagh Williamson RN, MSc

Chief Nursing Officer and COO

Formerly a nursing director, Creenagh’s career has involved emergency nursing and working in hospital and care settings. With a strong background in healthcare management and managed care, Creenagh oversees HN’s operations and leads a team of exceptional healthcare professionals.

Joachim Werr MD, PhD

Founder & Executive Chair

HN’s founder and Executive Chair is an entrepreneur, qualified emergency physician and has a PhD in Physiology from the Karolinska Institute, Sweden. Prior to HN, Joachim was vice president at Investor Growth Capital Limited – formerly a business unit within Investor AB. He also was an A&E Consultant at the Karolinska Institute and an engagement manager At McKinsey & Co, Sweden.

Mark England MBA

CEO

Prior to joining HN, Mark was Deputy National Director for UEC at NHSE/NHSI and Managing Director of a first wave Integrated Care System. He has overseen considerable transformation, much of it digitally enabled, within the NHS and beyond. Mark has worked at board level within providers, led systems and covered policy and strategy at a national level.

Prof. Matthew Cooke

Emergency Medicine Specialist

Matthew was formerly the urgent and emergency care ‘tsar’ for the UK Government, most known for his introduction of the four-hour target. He is also an experienced A&E physician, featuring twice on HSJ’s annual “top 100 clinical leaders”. His leadership roles have included National Director for NHS 111 and Chief Clinical Officer at Capgemini.

Steven is an influential and experienced GP, population health expert and clinical leader. He has a strong background in public health and clinical leadership. Steven practices as a part-time GP and as a freelance health consultant with his own consultancy company, Programmes for Health, supporting a range of healthcare commissioning and provider organisations.

Robert Harris was previously CEO of LHG, – an NHS England ‘Multispecialty Community Provider’ Vanguard organisation, and the largest primary care general practice partnership in the NHS. Robert also served as  NHS England’s National Director of Strategy and advised ministers on health policy and strategy. Robert brings strategic, governance and business growth  knowledge and capabilities to HN.

Sam is a serial entrepreneur and the Managing Director of a number of healthcare focused businesses. Having worked in management consulting, executive search and software sales he is well placed to support HN through the commercial landscape in health and social care. He is the current national co-chair for the UKs oldest children’s charity, Action for Children and has a passion for improving patient outcomes through innovation and the adoption of technology.

Graham Prestwich

Patient Director

Graham has participated in and led a wide range of patient and public involvement roles and activities over the last ten years bringing diverse and valuable experience and networks to his current role with HN. The experience includes Clinical Commissioning Group Lay Member for 6 years, Lead for patient and Public Involvement at the Yorkshire and Humber Academic Health Science Network, Board member of Healthwatch Leeds and Chair of the Bradford School of Pharmacy External Advisory Board. Graham also participates in several research projects as a lay member.

Heather Cook

Digital Health Architect and Market Access Specialist

Heather Cook’s recent experience includes successfully building the brand of Brain in Hand, a mental health digital support system. She has a background in consulting from PA Consulting and brings strategic and brand-building capabilities to HN.
