About us
HN was founded in 2010 by clinicians and researchers from the Karolinska Institute, Sweden. The UK operations were established in 2015 and we are proud to have helped improve the lives of over 30,000 patients by working alongside them to achieve better health.
We specialise in the practical application of population health management, by identifying high-cost, high-need patients, and supporting them to improve their outcomes and reduce their care consumption. HN has worked with the Nuffield Trust on a randomised controlled trial of AI guided clinical coaching within the NHS with excellent results.

The HN Story
Dr Joachim Werr, founder of HN is an NHS Innovation Accelerator Fellow and was previously an emergency physician and researcher in Stockholm.
Werr observed that the same patients would regularly attend A&E and so in 2010, he and his fellow researchers developed a first-generation statistical algorithm to track patients and assess their risk of returning to A&E. Attention was then turned to supporting the identified patients through telephone-based health coaching. Werr and his team soon discovered that supported patients did engage better with their own health and that it saved them from hospital admissions.
This was the beginning of the HN story and the foundation of our vision – to empower patients, by engaging them in their own health and care, thereby reducing their need for NHS services and creating sustainable healthcare for all.
HN’s journey in the UK began in 2015 with a large clinical trial with the Nuffield Trust, one of the UKs most prestigious healthcare research foundations. The trial demonstrated that the use of advanced data science and AI for screening high-need, high-cost patients, combined with digitally enabled patient centred programmes, significantly increased system efficiency, patient experiences and clinical outcomes.
From our UK launch, we have developed into an award-winning partner to the NHS and from March 2020, have been supported by the NHS Innovation Accelerator to scale nationally.
In addition to Joachim Werr, HN is led by CEO Mark England and Chief Nursing Officer & COO Creenagh Williamson. The company now has close to 40 employees across England, all of whom are dedicated to supporting our clients and their patients, the HN vision and the NHS Long Term plan.
We are proud to have a strong AI and data scientific team, digital product team and clinical leadership, governance and quality team. Our Advisory Board hosts national key opinion leaders.
Our goal is ambitious – to embed our services into NHS routine care within five years. For high-need patients, this will mean that ‘healthcare finds you before you feel the need to seek unplanned care’.

Our team
HN is comprised of professionals from a range of clinical, digital, data and business backgrounds. We are highly experienced in delivery of innovative clinical services, use of applied AI within healthcare, research and evaluation.
Most of the HN team have worked within the NHS so are familiar with its processes and all our patient facing services are delivered by trained and qualified healthcare professionals. Furthermore, we have an excellent Advisory Board and Regional Advisers who are senior stakeholders from within NHS commissioning, primary care, population health management, adult social care and research, that we actively work with to achieve our goals.
