We’re delighted to have won the SME Innovation award with our partner, L2S2 Ltd. Thank you HTN!

Our work involves applying AI to near real-time data, which identifies patients with the highest risk of unplanned hospital admission. These patients can receive digital health monitoring technology from L2S2 Ltd and a holistic assessment by HN Clinical Coaches. Together this provides a personalised care plan with regular coaching sessions, empowering patients to better understand their condition(s), develop self-management and self-monitoring capabilities and ultimately improve their outcomes.

Our work continues to help the NHS during the pandemic and with L2S2 Ltd, we have developed HN Covid Plus – a turnkey digital and staffed solution to support Oximetry@Home. This supports self-recording SpO2 readings and remote monitoring via a virtual ward board, including administrative support to ensure patients are safely onboarded. Further details can be found at the HN Covid Plus webpage.

We, with L2S2 Ltd really appreciate our work being recognised by the HTN Awards 2021 panel and congratulate all the finalists, highly commended and winners. This is a great platform to showcase everyone’s important work.
