Our work involves applying AI to near real-time data, which identifies patients with the highest risk of unplanned hospital admission. These patients can receive digital health monitoring technology from L2S2 Ltd and a holistic assessment by HN Clinical Coaches. Together this provides a personalised care plan with regular coaching sessions, empowering patients to better understand their condition(s), develop self-management and self-monitoring capabilities and ultimately improve their outcomes.
Our work continues to help the NHS during the pandemic and with L2S2 Ltd, we have developed HN Covid Plus – a turnkey digital and staffed solution to support Oximetry@Home. This supports self-recording SpO2 readings and remote monitoring via a virtual ward board, including administrative support to ensure patients are safely onboarded.
