Our work is centred around you. We take time to learn about how you live with your health conditions and provide a comprehensive support package to help you meet your health and wellbeing objectives and improve your quality of life.

Our dedicated health coaches are experienced nurses and skilled healthcare professionals. Over a period of typically between 4-6 months, they will work with you to understand your health conditions and give you the skills, confidence and support to improve your health and wellbeing.

How and why have I received a letter? Do you work with my GP?

We work with your NHS doctor who has identified you as someone who may benefit from our health coaching service.

What is health coaching?

Health coaching is delivered by one of our registered healthcare professionals over the telephone. Coaching takes the form of personal support and guidance on health and wellbeing matters of importance to you.

How does HN work with my GP and other health professionals?

We complement and work with all the professional people involved in your care.

What can I expect from my HN health coach?

Your health coach will work with you to understand what is important to you and to assist you to achieve your health and wellbeing goals.

How does the programme work and what is my role?

Your health coach will call you on the telephone at a time convenient to you to discuss those health and wellbeing issues of importance to you. We will agree a coaching plan with you over the coming weeks and months to support your progress.

What are the benefits of joining the health coaching programme?

We hope you will enjoy your time with us and your personal health coach. The majority of our patients report feeling more confident, empowered, better informed and happier with their health and wellbeing choices following their programme.

How will I meet with my health coach?

This is a telephone-based service to provide you with flexibility and to allow you to receive your health coaching at a time and location convenient to you.

Is information about me held securely and confidentially?

Yes, we hold all your personal information securely and confidentially in line with Data Protection laws.

Do you share information with my doctor?

To provide you with the best service possible, we will ask for your consent to share information with your doctor.

How do I get in contact with the HN team to begin my programme?

You will be provided with your health coach’s contact details when you join the service.

Is the service free and can I leave at any time?

Yes, there is no cost to you as it is covered by the NHS. You can end your health coaching programme at any stage.
