Health Navigator delivers proactive population health management services for commissioners, providers, and pharma.
We can help you to: equitably identify your priority patients using our proprietary, clinically validated machine learning solutions or by applying your preferred case finding methods; provide teams of accredited Clinical Coaches to deliver proactive patient activation services; and enable you to “close the loop” with real-time population health insights and outcome evaluation.
Patients report a much more positive care journey and our customers report better clinical outcomes and more cost-effective delivery of care.
Health Navigator has spent over 10 years researching and delivering proactive population health management services for the NHS. We have worked with data for 10 million people from 30 acute hospitals to deliver case finding solutions and have provided clinical coaching to patients within 100 neighbourhoods.

Our Founder - Dr Joachim Werr - established Health Navigator in London in 2014 based on his front-line experience and academic research.
Joachim was working as an A&E doctor and researcher at the Karolinska Institute when he noticed that he kept seeing the same patients with long term conditions enter crisis and appear on his unit. Intrigued, Joachim started taking patient histories and analysing hospital records.
Joachim made three discoveries. First, that a small number of patients use 90% of unplanned hospital bed days every year. Second, that the individuals in this “high consuming” group change every year but can be identified using machine learning before they relapse. Third, that clinical coaches can help these patients dramatically to reduce the length of hospital stays.
Based on these fundamental insights, Joachim moved to London and setup Health Navigator in 2014.
Joachim has been continuously involved in the organisation since it's foundation. Currently he is Chair of the Board of Directors as well as being directly involved in ongoing scientific research.
The NHS' Ten Year Health Plan challenges us all to “put power back in the hands of people and professionals to make the best choices.” Health Navigator enables patients to take control of their health and offers Integrated Care Boards and providers an enterprise scale partner who can enable them rapidly to shift care, and resources, “left”.
