We’ve spent 10 years developing and clinically validating an artificial intelligence algorithm that can identify these patients. We’ve worked with communities across the UK to develop the algorithm and it is also validated and adjusted for performance and health equity every time it is deployed.

Our NHS Personalised Care Institute accredited clinical coaches then offer the patients identified by the model personalised coaching to help them take control of their health and stay out of hospital.
